Terms of Service

By creating an account or using Surforme, you agree to these terms.
To use Surforme, you must reside in Québec and be of the age of majority.
Surforme organizes and synthesizes your health information into a structured Portrait and surfaces patterns and gaps so you can have better conversations with qualified professionals. Surforme does not provide medical advice, does not diagnose, does not predict outcomes, does not interpret scans, and does not prescribe treatment, and it does not replace your physician or care team. In an emergency, contact emergency services.
Signing in uses a secure email link, so there is no password. You are responsible for access to your email and for activity on your account.
These billing terms apply once paid plans open; nothing is charged while Surforme is at the waitlist stage. Plans are offered on an annual commitment, billed annually, with renewal disclosed before each cycle. You can cancel before your renewal date to prevent the next billing cycle.
Use Surforme only for your own health information. Do not misuse the service, disrupt it, or try to access it unlawfully.

The Surforme platform, its design, and its content are owned by Surforme Inc.
The service is provided on an as-is basis. Surforme does not warrant that a synthesis is complete or free of error, and it is not a substitute for professional medical judgment.
To the extent permitted by Québec law, Surforme is not responsible for indirect or consequential damages arising from your use of the site or service. Detailed limits will be set out before paid plans open.
These terms are governed by the laws of the Province of Québec and the federal laws of Canada that apply there.
We may update these terms. When we do, we will revise the effective date above and communicate any material change.
